Resolution:

Salesforce Exception from Swarm Request for Slack “Your org is missing a Service Channel for this Swarm Member, or skills, or both. To run Swarming flows, set up Omni-Channel and add both the Service Channel for the Swarm Member and required skills.” is thrown when the Service Channel for Swarm Member object/entity is not created.

To fix the exception, create a Swarm Member Service Channel. Go to Service Channels in the Salesforce Setup to create the Service Channel for the Swarm Member object/entity.
